Subject: Mouth Ulcers

Hi, I have tried to post once but it didnt work so am trying again so forgive me if I have done it wrong.
I am new to the forum and I have rheumatoid arthritis and am wondering if anyone has suffered large mouth ulcers, I have also got raised white patches, which are quite large on the inside of my cheeks. I have been on methotrexate for 9 weeks and my dose is now 20mg weekly. I also take folic acid the day after. I have just read online that this could be a sign of toxic reaction to the methotrexate, any help would be appreciated as I am still learning to live with this illness.

I'm afraid mouth ulcers are one of the side effects of MTX. If I was you I would ring up my specialist nurse and tell him/her, they will probably tell you to increase your folic acid, this should help. When my liver function test went up mine was raised for one a week to one everyday.

Hi Julie , Mouth Ulcers are caused by Methotrexate & one reason we take folic acid with it , they have increased your Mtx pretty quick to get to 20mg , I have been on the drug a long time & its very good for RA but would recommend a high dose of folic acid with it , I take 5mg every day apart from the day I take 25mg Mtx , it will feel better if you can get your folic acid increased

thankyou for replies, I actually went to g.p. this morning, he couldnt believe the size of the ulcer its as big as a 10p piece - gosh it hurts. When I was first diagnosed my g.p. gave me 5mg folic acid to take one a day except on day of methotrexate which to me was fine, but when I went to the rheumatology clinic couple of days later the doctor I saw said certainly not, you take one a week and that is the day after methotrexate, on no account do you not take daily,she is a bit scarey, so I did as she said. Today however after talking to my g.p. who wasnt aware of this, said that I have not had sufficient folic acid all these weeks, I have only been taking 5mg, so he has put me back onto one a day, plus some spray for my ulcer (which seems a bit better). Maybe this is why I have felt so ill for 3 days after taking Methotrexate.

Not only that but last two weeks tablets were wrongly written on tub by pharmacist, lucky I noticed that.

The script I picked up this week also had a mistake, for some reason a nurse from the hospital had written to my g.p. saying my dose should be 18 tablets, take 9 once a week 20mg x 2.5 tablets. (I only take 8 a week) Again I noticed and realised so g.p. had to redo script for me, he has complained about this to the hospital and we still dont know why the nurse sent the note in the first place as I havent had an increase or seen any one at the rheumatology department. It is worrying actually because our local hospital is not a place where you really want to go if you dont have to. My original consultant which in the beginning was a private consultation, has his own NHS clinic and he told me that if I didnt like anything about the clinic where I had to attend or anything about my treatment then I had to ring his secretary and he would put me into his own clinic, bit further to travel but he is a brilliant and highly respected consultant in his field, so I just might do that. As I said I am still learning how to deal with this illness, but I would really like to think the medical staff know what they are doing, thank goodness for forums like this where people can give you really good advice. Thanks again for help.
