Subject: RA and medical declaration forms

Hi everyone,

I'm about to start a new job in a week or so and have a medical declaration form to fill in beforehand. Even though this job is not a physical one I am concerned that it may put off potetntial employers hiring me even though they should not discriminate in this way. Any thoughts on how to deal with medical declaration forms? Any stories that can help shed some light on how to deal with such forms? The job is a clinical one that involves phone and computer work.
