Subject: Struggling to calm a flare up

I have been on methotrexate 7 weeks on 10 mg, and I'm due to start 15mg next week. In the meantime I have been off work six weeks as I am having a bad flare up. I take tramadol, lodine and lansaprazole and folic acid, I have bought a tens machine which works when I use it most of the time but the aches and pains return as soon as I turn it off. I am at the end of my tether as to know what to do next and I am due back at work on monday which involves hours of walking every day and 8,9 and 10 hour days. The methotrexate just isn't doing anything and I will have to wait even longer to see if the new dose will help. Sorry to moan but it's getting to me now.... any ideas? Sharon

Hi Sharon.
So sorry that you`re struggling with pain. Have faith in the methotrexate, it just takes time.. The only thing I can think of is that you get some ibuprofen because they`re anti-inflammatory but you need to ask your pharmacy to make sure they don`t interact with anything you`re taking already.

the one thing that worked for me for about 4 weeks was a second very large steroid injection, definetly got me through work until the dmards kicked in. is that something you could be offered or would be able to take. I couldn't get relief from painkillers or antimflammatories.

Hi Rich, I have tried ibuprofen in the past but not since being on methotrexate. I'm ringing the rhummy nurse tomoz so I will ask if I can take it with everything else. Thanks Sharon

I find it difficult to get hold of my rheumy nurse at times, hence the ibuprofen suggestion and know that when things are really bad, ibuprofen sounds a bit weak.
I`ve had a large steroid injection in the past during a big flareup and it worked like a miracle, so hope they sort you out.

Hi Sharon you need a good 3 -4 months on methotrexate before you will get any benefit, but you will, it took a long time with me. you do know it wont help much with the pain it realy just slows down the disease. I think you need a better pain med than the one your on good luck. take care x

I too am on methotrexate, only 7.5 mg, but for months now I have had terrible shoulder and neck pain at night. It eases in the morning but I cannot lift my left arm very high. My GP and cons have both suggested I take paracetomals for the pain but I'm reluctant to take them every night, but the pain and the sickness I feel means I get very little sleep. I work full time so it really gets me down.

Give MTX time...it does its job in that it calms down your over active immune system.

Take paracetemol regularly.. do not wait for the pain.
